Sacks is a specimen. Curiously, the tongue poking out of his mouth has a faintly feminine air to it, but it enhances, rather than diminishes, his masculinity. A complementary counterpoint, if you will. It reminds me of a young Marlon Brando, whose sexual proclivities (both ways) are no secret.

Indeed, Doctor, Israel continues to produce fine, strapping sons. One has to wonder if the prohibition against man-sex was to keep the men from enjoying each other too much! :)
